We were very happy to accept your preliminary registration for NAPLE at the founders meeting on 4. October last, but we now need to move on to a more formal confirmation of this.

Would you therefore be kind enough to fill in the enclosed form and indicate who is the formal member of your organisation and then send us this information as a fax with his/her signature.
